When was your last comprehensive (medical) eye exam? Were you dilated? Did you have an injury/concussion/stroke since your last eye exam?
Need the best baseline/up-to-date prescription for a binocular vision exam and to rule out eye health conditions that could impact binocular vision, which can be done with Dr. Sieg or Dr. Wallerich at our partner clinic, Swoop Eye Care, through your medical insurance/worker’s compensation or auto insurance, which is located in the same building as NeuVision Therapy & Rehabilitation.
If has been greater than 1 year since the last exam – need an updated medical eye exam first.
If had an exam within 1 year, request copy of exam notes – need PRIOR to being seen for binocular vision exam.
If had a new concussion/injury/stroke since last eye exam – recommend re-evaluated as medical eye exam or office visit prior to binocular vision exam.
After medical eye exam (or receiving last eye exam records via fax/email) then schedule for a binocular vision examination (BVE) - this will allow us to access your eyes strength, endurance and flexibility of your binocular vision at distance and near to determine if you are a good candidate for vision therapy or rehabilitation and what the duration of it will likely be.
This also can be done through our partner clinic, Swoop Eye Care, and billed to your medical insurance/workers compensation or auto insurance.
After your BVE, your neuro-optometrist will offer vision therapy & rehabilitation or sports vision therapy options if appropriate through NeuVision Therapy & Rehabilitation.
It is an out of network service. Only accept cash, credit card, HSA/FSA (and soon auto insurance and workers compensation).
You may try to self submit to your medical insurance company.
